South Jersey Hospital-at-Home Programs Resume Amid Uncertain Future
In South Jersey, hospital-at-home programs are resuming operations after a brief suspension due to a government shutdown. These programs, part of a federal initiative known as the Acute Hospital Care at Home, allow eligible patients to receive hospital-level care in their own homes. This innovative approach includes remote monitoring and in-person treatments such as blood draws and X-rays, providing a viable alternative to traditional hospital stays.
Resuming Services with a New Challenge
Patients arriving at a Virtua Health facility now may choose between a hospital stay or receiving treatment from home, a flexibility that has garnered interest since its inception during the COVID-19 pandemic. Dr. Diego Ortega, the clinical lead physician of Virtua’s Hospital at Home program, emphasizes the program’s potential, stating, “It truly is an acute care program that brings all the resources of the hospital room that you traditionally imagine right into your own living room.”
While Virtua Health and other participating health systems have resumed enrolling new patients after the government shutdown ended on October 1, they face an impending expiration of the program at the end of January 2024. Health care providers are advocating for Congress to either make the program permanent or to extend it for an additional five years.
Benefits and Success of Home Care
The Acute Hospital Care at Home initiative, launched in 2020, was designed to alleviate pressure on hospital systems by allowing patients with manageable conditions to receive care in a more comfortable setting. According to the Centers for Medicare and Medicaid Services, approximately 147 health systems across the United States are now approved for participation, with over 400 facilities involved.
Dr. Rupa Kashyap, chief hospitalist at Virtua Willingboro Hospital, highlights the program’s effectiveness, noting that patients treated at home had lower mortality rates and fewer hospital-acquired infections compared to those receiving care in traditional hospital settings. “Whether they have less hospital-acquired infections, they have less falls, they have less episodes of confusion… all that is much less when they are cared for at home,” Kashyap added.
Eligible candidates for the program include individuals suffering from conditions such as COVID-19, pneumonia, and congestive heart failure. Patients receive continuous monitoring through devices that transmit vital signs to a command center staffed by health professionals. This real-time data allows for immediate adjustments in care, enhancing the overall treatment experience.
As the program enters its next phase, the growing interest from both patients and insurers signals a shift in how health care is delivered. Initially limited to Medicare patients, Virtua Health has since partnered with private insurers to expand coverage options.
Despite the promising trajectory, the future of the hospital-at-home model remains uncertain. The recent government shutdown has complicated matters, with the Medicare waiver expiring on January 30, 2024. If Congress does not extend or permanently adopt this initiative, health systems may have to revert to traditional hospital care for their patients.
The timing is particularly concerning, as hospitals typically experience increased patient volumes during the winter months due to respiratory illnesses. Dr. Kashyap pointed out that the surge in hospital admissions during this season further underscores the importance of maintaining flexible care options. “Every winter, we see surges in the hospital. We see the ERs getting full,” she remarked, highlighting the need for an effective solution to manage patient care during peak times.
As these programs navigate their uncertain future, the potential for home-based care to revolutionize patient treatment and outcomes remains a focal point for health care providers and policymakers alike.
